Testimonial Client taking BPFoundations of East ChicagoBridges to Wellness Network (BTWN) is changing the lives of countless East Chicago citizens. BTWN is an alliance between HealthVisions Midwest, Healthlinc, St. Catherine Hospital, and a growing numbers of partners made possible by Foundations of East Chicago’s Collaboration Grant. This network of health care facilities has banded together to eradicate poor health amongst citizens of East Chicago 45 years of age and older. Patients receiving care from St. Catherine Hospital, HealthLinc or any number of affiliates will be referred to BTWN. This program focuses on disease prevention, health education and nutrition and fitness information and resources. A few of the available referrals are listed below:

 

  • Nutrition classes
  • Cooking classes/healthy eating
  • Matter of Balance
  • Well-Walkers  or other walking opportunities
  • Tomando
  • Self-monitoring stations
  • Chronic disease self-management classes
  • Tobacco cessation classes
  • Mental Health First Aid tools used to re-establish health amongst East Chicago’s middle aged to elderly population.

“My doctor considered starting me on blood pressure meds. The BTW Community Health Workers encouraged me to exercise and gave me tips on healthy eating. As it turns out, my anxiety has decreased and my blood pressure levels are normal. I attribute this to the motivation and encouragement I received from BTWN”.
